POST OPERATIVE EXAM: Day/date/time
PRE-OP INSTRUCTIONS FOR LASER-ASSISTED IN-SITU KERATOMILEUSIS (LASIK):
1.) Please purchase over the counter “Preservative Free” artificial tears (any brand ok) and fill the prescription eye drops
that were given to you. Please bring these with you on the day of your surgery.
2.) Please fill out the Informed Consent form and bring this with you on the day of surgery.
Please do not sign the form until you meet with a Patient Counselor on the day of your surgery.
3.) For all cases, it will be necessary for you to be seen by the doctor within 48 hrs. Your post-
operative appointment MUST be kept.
4.) YOU MUST HAVE SOMEONE DRIVE YOU HOME AFTER SURGERY. We cannot assume liability
for any vehicle left in the parking lot.
5.) Absolutely no makeup around the eye on the day of surgery and for one (1) week after
surgery. This includes sunblock, and moisturizers.
6.) Wear warm comfortable clothing and walking shoes.
7.) Please inform the doctor of any prescription medications you are currently taking.
8.) No restrictions on meals. Do not consume alcohol beverages on the day of surgery.
9.) You may be given 5 mg to 15 mg of Valium orally in the office on the day of surgery. If you prefer to
not take the Valium, please let us know.
10.) CONTACT LENS WEARERS: remove 24 hours/1 week prior to surgery
If you need to make any changes to your appointments, please give us at least 48 hours advance notice.
