Laser Eye Center Exam & Surgery Confirmation (SJ)

COMPREHENSIVE EYE EXAM: Day/date/time

SURGERY: Day/date/surgery time
POST OPERATIVE EXAM: Day/date/time

The exam appointment will take approximately 1 1/2 hours.
It involves a corneal mapping, corneal thickness measurements, topography and a consult with Dr. Bindi. 
The exam is free of charge if you have not had previous eye surgery. 
If you wish to obtain a prescription for glasses there is a charge of $85. 
 
REGARDING CONTACT LENSES: For the most accurate screening measurements, Dr. Bindi prefers that you
remove soft contact lenses 7 days prior to your appointment -or- hard/gas permeable 3 weeks prior.  
PRE-OP INSTRUCTIONS FOR LASER-ASSISTED IN-SITU KERATOMILEUSIS (LASIK):
1.)  Prescriptions for eye drop medications will be provided. Please fill all required prescription eye drop
      medications prior to your surgery(antibiotic drop, anti-inflammatory drop).  Purchase over the counter 
      “preservative free” artificial tears (any brand ok). Please bring these with you on the day of surgery. 
2.) For all cases, it will be necessary for you to be seen by the doctor within 48 hrs. Your post-
     operative appointment MUST be kept.
3.) YOU MUST HAVE SOMEONE DRIVE YOU HOME AFTER SURGERY.  We cannot assume liability
      for any vehicle left in the parking lot.
4.) Absolutely no makeup around the eye on the day of surgery and for one (1) week after
     surgery. This includes sunblock, and moisturizers.
5.) Wear warm comfortable clothing and walking shoes.
6.) Please inform the doctor of any prescription medications you are currently taking.
7.) No restrictions on meals. Do not consume alcohol beverages on the day of surgery.
8.) You may be given 5 mg to 15 mg of Valium orally in the office on the day of surgery. If you prefer to
      not take the Valium, please let us know.
9.) CONTACT LENS WEARERS: remove 7 days prior to surgery.
If you need to make any changes to your appointments, please give us at least 48 hours advance notice.
